The documentation output of controls testing is a test workpaper: the control tested, the testing methodology, the sample selected, the evidence reviewed, and the conclusion. Why Conflation Occurs - and Where It Breaks Down

A Separated Methodology
A methodology that properly separates the two activities has distinct phases, distinct teams (or at least distinct roles), and distinct documentation outputs.
Phase 1 - Obligation Extraction: Extract every obligation from the regulatory text. This is best done by a specialist with regulatory reading experience - not a generalist controls tester. Each obligation is given a unique identifier and stored with the source paragraph reference.
